Our Model | Physician-Led Lifestyle Medicine
See the Pattern. Change the Behaviour. Improve the Outcome.
Data-Driven Behaviour Modification for Sustainable Metabolic Health.
New Me Health combines physician guidance, Remote Patient Monitoring (RPM), Continuous Glucose Monitoring (CGM) where appropriate, evidence-based Lifestyle Medicine and strategic behavioural coaching. RPM and related assessments may include body weight, body composition, blood-pressure markers, fitness tracking and periodic 3D body scanning, depending on the programme, clinical need, eligibility and availability. Together, these elements help participants connect everyday choices with measurable health patterns and turn personalized insight into sustainable action.
Start with a conversation about your health, goals and the support that may be appropriate for you.
Beyond the Repeating Cycle
When Managing the Condition Is Not Creating Enough Health
Medication, procedures and specialist care can be essential. Yet many people living with obesity, insulin resistance, prediabetes, Type 2 diabetes, high blood pressure and related metabolic conditions remain caught in a repeating cycle of appointments, laboratory tests and treatment adjustments.
The missing piece is often not another piece of information. It is a structured way to understand how daily patterns — including food, movement, sleep, stress, relationships and risky substances — interact with clinical risk.
Our model complements appropriate medical care by connecting clinical insight with practical action. We help participants understand what may be influencing their health, decide what to change first and receive support as they turn those decisions into repeatable habits.
Address contributing causes — not symptom management alone.
Measure What Is Happening. Understand Why. Change What Happens Next.
01 | The Gap
Most people already know that food, movement, sleep and stress influence health. The difficulty is seeing how their own behaviours affect their own body — and knowing which change will matter most. New Me Health closes that gap through Data-Driven Behaviour Modification.
02 | RPM
Remote Patient Monitoring (RPM) uses approved connected devices and programme-relevant assessments to make important health and behaviour patterns more visible over time. Depending on the programme, clinical need, eligibility and availability, this may include body weight, body composition, blood-pressure markers, fitness and activity tracking, and periodic 3D body scanning. These measurements can help the care team and participant observe change, recognize barriers and make more informed adjustments.
03 | CGM
Continuous Glucose Monitoring (CGM) provides glucose data across the day and can show how meals, activity, sleep, stress and timing may influence individual glucose responses. CGM is used where clinically appropriate and available; it does not replace diagnosis, prescribed treatment or professional clinical judgement.
04 | Coaching
Data alone does not create change. The New Me team helps participants interpret relevant patterns, connect them with everyday behaviours and identify practical adjustments. Strategic behavioural and Lifestyle Modification coaching then supports goal setting, problem solving, accountability and repetition until healthier actions become more consistent.

Intensive Therapeutic Lifestyle Change
A Structured Model for Meaningful and Sustainable Progress
Our physician-led Intensive Therapeutic Lifestyle Change model, or iTLC, translates evidence-based Lifestyle Medicine into a personalized process that participants can apply within the realities of work, family, culture and existing medical care.
It is not a crash diet, a generic wellness challenge or a substitute for necessary treatment. It is a coordinated approach that combines education, health insight, goal setting, practical skill-building, accountability and clinical awareness.
Depending on the individual, the model may support improvements in weight, blood sugar, blood pressure, insulin resistance, cholesterol, sleep, energy and other markers of metabolic health. The pace and degree of progress vary, and no specific outcome is guaranteed.
Physician-led. Evidence-based. Personalized. Measurable. Sustainable.
The Foundations of Lifestyle Medicine
Six Areas That Work Together to Influence Health
The model considers six established pillars of Lifestyle Medicine. Participants are not expected to change everything at once. Priorities are selected according to clinical needs, readiness, safety and personal circumstances.
01
Whole-Food Plant-Predominant Nutrition
Build a realistic eating pattern centered on minimally processed foods and greater plant variety, adapted to health needs, culture, preferences and clinical considerations.
02
Regular Physical Activity
Develop safe and progressive movement habits that support strength, mobility, cardiovascular health, insulin sensitivity and daily function.
03
Restorative Sleep
Identify routines and barriers that affect sleep quality and duration, and build habits that support recovery and metabolic health.
04
Stress Management
Recognize stress patterns and practice sustainable strategies that support emotional regulation, recovery and healthier decision-making.
05
Positive Social Connection
Strengthen supportive relationships and reduce isolation because connection, belonging and accountability influence long-term health behavior.
06
Avoidance of Risky Substances
Reduce or avoid tobacco and other harmful substance exposure while supporting safer choices and appropriate professional care when needed.

Your Starting Point Shapes Your Path
Metabolic health is influenced by medical history, medications, sleep, stress, food access, work schedules, family responsibilities, physical ability, culture and readiness for change. For that reason, our model is personalized rather than built around a single rigid formula.
Where appropriate and available, progress may be considered through measures such as weight, waist-related risk, blood pressure, blood glucose, HbA1c, lipid markers, sleep, energy, functional capacity and medication use. Not every measure applies to every person, and interpretation remains within the appropriate clinical context.
When RPM, CGM or other monitoring tools are used, the purpose is not surveillance for its own sake. The purpose is to make relevant patterns more visible, support better-informed coaching conversations and help participants test which practical changes are working for them.
Progress is not defined by perfection. It is defined by informed action, safer self-management and changes that can be maintained.
Safety First
Lifestyle Change Must Work with Appropriate Medical Care
New Me Health programs complement — not replace — primary, specialty, urgent or emergency medical care. Participants should keep their healthcare providers informed and continue prescribed treatment unless a qualified clinician directs otherwise.
As blood sugar, blood pressure, weight or other health indicators change, medication needs may also change. Medication reduction or discontinuation is never automatic and must occur only under qualified clinical supervision.
Connected-device readings, CGM trends and remote reports can support clinical awareness but do not replace diagnosis, emergency evaluation or the judgement of the participant's qualified healthcare professionals.
Symptoms suggesting an urgent or emergency condition require immediate evaluation through the appropriate medical service. Educational content, coaching and masterclasses are not emergency-care services.
Never stop insulin or another prescribed medication based only on website content, a class, a testimonial or another participant's experience.
One Word Difference Can Change the Medical Meaning
Type 1 and Type 2 Diabetes Require Different Positioning
Type 2 Diabetes
For people living with insulin resistance, prediabetes or Type 2 Diabetes, structured lifestyle change may support meaningful improvement in metabolic health. In appropriate cases, some individuals may work toward remission under qualified medical supervision. Outcomes vary, and medication changes require clinical oversight.
Type 1 Diabetes
Type 1 Diabetes cannot be reversed through lifestyle change, and insulin remains essential. Lifestyle support may complement specialist care by helping individuals strengthen nutrition, activity, sleep, stress management and day-to-day self-management. With individualized medical guidance, these efforts may support insulin sensitivity, overall metabolic health and safer management aimed at reducing avoidable DKA and long-term complication risk.

Priorities reflect the participant's condition, risks, goals, circumstances and readiness.
06
Structured
Education is connected to practical steps, accountability and progress review.
07
Coordinated
The model complements ongoing care and respects the role of primary and specialist clinicians.

Progress Is Personal
Better Numbers Matter. So Does a Better Life.
Depending on the individual, progress may include improved health understanding, more consistent daily habits, greater energy, better sleep, improved confidence, healthier weight trends, improved blood sugar or blood pressure, greater functional capacity or reduced medication needs under clinical supervision.
Some changes may appear quickly; others require sustained effort. Outcomes depend on medical circumstances, participation, adherence, biology and other factors. No single timeline or result applies to everyone.
Real patient experiences can help visitors understand what meaningful change may look like.
